When your physiatrist approves your discharge, your case manager will help you and your family prepare for
discharge from the hospital -- coordinating family training and meetings with your care team, arranging for
rental or purchase of equipment you will need at home, locating supportive services and resources, and
arranging for additional therapy, if needed.

from additional therapy, and it’s comforting to know SSM Rehabilitation Hospital’s services can transition with

Whether you need therapy one day a week or several days a week after leaving the hospital, SSM Rehabilitation Hospital provides outpatient rehabilitation at 33 convenient locations in the St. Louis area.
If you still require comprehensive rehabilitation, but not intensive therapy in the acute hospital setting,
SSM Rehabilitation Hospital’s Day Institute Program—with multiple locations—offers a specialized, transdisciplinary approach
to recovery in a flexible full-day format. Most patients participate in the SSM Day Institute five days a week. As a help to you
and your family, this program offers transportation services to and from therapy for a nominal fee.
And if you still need medical care and therapy, SSM Homeward Bound offers specialized rehabilitation centers within skilled nursing
and long-term care facilities to help you prepare to return home safely when ready. Nursing and therapy services are also available
in the comfort of your own home through SSM Home Care.

Helping prepare for the patient's return home
SSM Rehabilitation Hospital offers a free class twice a month for families and other caregivers. It is designed to help family members prepare for the patient's return home after inpatient rehabilitation.
The class is taught by SSM Rehabilitation Hospital occupational and physical therapists, and covers:
- Preparing the home
- physical modifications
- Equipment
- what you need, how to use it
- Home safety
- preventing injuries and falls for the patient and family caregiver
- Assistive devices
- tips for properly using them
- Community resources
- how to access help

- Opportunities to share ideas, concerns, and questions with other patients' families
in similar situations. The Caregiver Class is offered through the collaborative
efforts of SSM Rehabilitation Hospital, SSM Home Care, and SSM Rehabilitation Hospital Foundation.
